Here is how to easily access FORMED:

  • On your smartphone, tablet, or PC, go to your internet browser (Edge, Chrome, etc.) and type in the address line: www.FORMED.org

  • ​

  • Scroll down to the middle box: SIGN UP, and select "Sign Up as a Parishioner"

  • ​

  • In the "Search Your Parish/Organization" Box, type our Parish zip code: 14616

  • ​

  • Select the option that says:  St. Charles Borromeo - 3003 Dewey Avenue, Rochester, NY 14616

  • ​

  • Enter your NAME and EMAIL you want to Login with (In the future, you will only need your email to gain access)
